This form is required for all campers and staff members at weekend retreats and for guest groups.  Please print this page and bring a signed copy to Camp with you at the start of the camp program.  Summer campers do not need this form because the Health Certificate includes the authorizations and signatures.  If you have questions about this form, please contact office@campelim.com.
